Please be advised, the state of Connecticut requires the following paperwork to be submitted for students files. All of the following must be submitted prior to finalizing enrollment:

• Proof of yearly physical
• Up to date immunization record including yearly flu vaccine
• Complete admissions packet
• Additional information will be required by CT for state funded placements. Please speak with our admissions staff for further information.
